Dear Residents and Families,
There have been many updates in restrictions for long term care and assisted living. Some of the changes that have occurred are in regards to visitation. Right now there are no restrictions to visitations. This means:
When visiting the facility during the hours of 5:30AM and 6PM, all visitors will need to check in at the 4th street entrance (Assisted Living side). All other times (6PM-5:30AM) visitors will need to go to the 3rd street entrance (Care Center side) and ring the doorbell for assistance. If the facility notices an increase in visitors or issues with needed space for visits, we will notify families and residents about the possibility of scheduling visits. So far there has not been an issue and we do not anticipate issues. If you plan to visit on a holiday it might be a good idea to call ahead, but at this time that is not required. We are excited for our residents to enjoy visits with fewer restrictions.
Resident quarantines have been updated as well. The only times a resident will need to quarantine, in regards to COVID are:
Please notify the facility if you become positive for COVID or symptomatic within 72 hours of visiting our facility.
Masks will still be required in healthcare settings, even when they are no longer mandated in the community. We thank you in advance for your continued cooperation and dedication to keeping our residents safe. If you have any questions, please do not hesitate to call.

We are NO LONGER in outbreak status. We have NO active cases of Covid (staff or residents) and we continue twice weekly testing for all employees per regulation. Cases are dropping community-wide and we are open to visitors and plan on resuming group activities and dining this week.
We are in outbreak status in our Assisted Living facility and are taking all required steps to prevent the spread. We will continue with testing staff and residents until we are all clear with our county health department and have no new or developing cases.
At this time we are not allowing visitors in our assisted living, but are allowing TV visits in our nursing home. Our nursing home is located in a different physical location and we are safe to have nursing home residents have visitors at this time. Our assisted living residents are not able to have visitors at this time due to the outbreak.
We had one more staff member test positive yesterday, but we are continuing visitation until more cases are found. Dining and activities resume on the 17th pending new cases.
All residents have tested negative and vaccinated residents can have visitors no. Starting on 1/17, all unvaccinated residents can have visitors. No communal dining or group activities until 1/17 if there are no new positives.
We tested all residents today and they are negative. We will test again on Monday. We have 8 total staff positives and will test staff again tomorrow.
We had two positive staff members earlier this week on our Assisted Living, but we’ve tested all residents today and there were no positives. We will test again on Monday and if there are no new cases, we’ll be able to reopen and have visitors again. We won’t be able to have communal dining at that time though.
Hello All,
We have clearance from Cowlitz County Health Department to resume admissions, group activities, communal dining, and visitations. We will continue to be under observation for the next two weeks by the health department. If for some reason we do end up getting another positive during that time will just extend the outbreak status. Activities is resuming groups today. We need to work on the dining rooms and get things arranged before we start communal dining.
We continue ongoing testing for residents and staff during our outbreak. We are following all precautions to avoid continued exposure and are now seeing cases decline and residents recovering. We will make an update when we are able to have visitation again. Thank you for your continued patience.
